Trichomoniasis
Trichomoniasis, sometimes referred to as "trich", is a common cause of vaginitis. It is a sexually transmitted disease. It is caused by the single-celled protozoan parasite Trichomonas vaginalis.

Trichomoniasis is primarily an infection of the urogenital tract; the most common site of infection is the urethra and the vagina in women.

Approximately 174 million people worldwide are infected with this parasite each year, making it the most common curable sexually transmitted infection worldwide

The average size of a trichomonad is 15 mm (they are not visible with the naked eye).

Symptoms

Typically, only women experience symptoms. These include inflammation of the cervix (cervicitis), urethra (urethritis), and vagina (vaginitis).

Women

• Vaginal discharge
• Vaginal itching
• Smelly, itchy, and typically frothy or foamy discharge
• Yellow or gray-green discharge
• Pain with urination possible

In rare cases, lower abdominal pain can occur. Symptoms usually appear in women within 5 to 28 days of exposure. While symptoms are most common in women, some men may temporarily exhibit symptoms such as

Men

• The majority of infected men have no symptoms
• Urethral discharge
• Pain with urination
• Pain and swelling in the scrotum (from epididymitis)

Diagnosis

Trichomoniasis is diagnosed by visually observing the trichomonads via a microscope. In women, the doctor collects the specimen during a pelvic examination by inserting a speculum into the vagina and then using a applicator to collect the sample.

The sample is then placed onto a microscopic slide and sent to a laboratory to be analyzed.

Causes and Risk Factors of Trichomoniasis

Trichomonis can be transmitted through sexual intercourse. While trichomoniasis is usually passed sexually, it may be picked up from contact with damp or moist objects such as towels, wet clothing, or a toilet seat, if the genital area gets in contact with these damp or moist objects.Unlike most STDs, the parasite can live for about an hour on damp towels, washcloths, and bathing suits.

If someone uses these towels or washcloths or puts on the bathing suit, the disease may be passed on that way. The good news is that trichomoniasis is curable.

Complications

• Trichomoniasis may cause a woman to deliver a low-birth-weight or premature infant.
• Trichomoniasis is also associated with increased chances of cervical cancer
• Evidence implies that infection in males potentially raises the risks of prostate cancer development and spread due to inflammation.

Prevalence and prevention

Use of male condoms may help prevent the spread of trichomoniasis.

Medical Treatment

i. The treatment of choice is antibiotics prescribed by the doctor. In addition to antibiotics, some alternative medicine therapies are available.
ii. Home therapy should not be used instead of a doctor's visit and appropriate antibiotics, as this may allow the spread of the condition as well as harmful sexually transmitted diseases, such as syphilis, HIV, gonorrhea, or chlamydia. (The author does not advise you to use these alternative therapies but these are listed to be complete.)
iii. Some people feel that natural douches once a day while lying in a warm bath may help. To increase parasite-killing activity, you may add the juice of one lemon to the following douche:

Vinegar douche - 1 teaspoon vinegar to 1 quart warm water

Inguinal Itching
JOCK ITCH or INGUINAL ITCH

It is common fungal infection that effects the skin, inner thighs, buttocks and genitals. It also called as tinea cruris or ringworm of the groin and is one of the most common infections of groin area. This fungal groin infection mainly targets men and adolescent boys. Jock itch can be triggered by friction from clothes and prolonged wetness in the groin area, such as from sweating.

It mostly restricts itself to the creases of the upper thigh, and does not invade the scrotum or the penis. However, it may spread to the anal region and cause itching, pain and burning sensation.

Symptoms

Symptoms include Itching in groin, thighs, skin folds, or anus. There may be red, raised, scaly patches that may blister and ooze. The patches often have sharply-defined edges and are often redder around the outside with normal skin tone in the center. Abnormally dark or light skin is also symptom in this condition. Flaking, peeling or cracking skin in your groin. In long standing infections, the rash becomes less itchy and less red and plaques may fuse together.

Generally, people tend to ignore the symptom, then as it aggravates, there is a tendency to buy the creams from the chemist over the counter. This subsides the itching for some time but as the exact cause is not known, generally after some days, again the infection comes back. People keep on trying different creams for years together but they are shy to go to the doctor. There should be the scientific approach to tackle the situation. Seeking doctor’s help in time can cure the infection well in time and further complications can be avoided.

You must have seen, Males standing in a group, talking to their companions & unknowingly scratching their groins areas. Many times they do not know what they are doing, but for others it is very embarrassing particularly when females are there!

There is a continuous distraction due to the itching. The person cannot concentrate and thus undergoes stress. This may disturb the life of the sufferer. So, it is important that such infections should be treated in time.

CAUSES OF JOCK ITCH: –

• Fungi causing jock itch is present on human skin and feed on dead skin. The moist and damp area provides them a perfect environment to grow and breed, particularly people having active life or those who sweat a lot suffer with jock itch frequently.
• People having been weaker immunity or suffering from diseases like diabetes HIV/AIDS are prone to suffer great jock itch.
• People sharing towels, clothes and people involved in activities where they cannot keep groin areas dry most of that time like sportsperson and people in sitting jobs are also at risk of catching jock itch.
• People suffering from other types of fungal infections like athletes foot may also suffer from this condition.
• Infection caused by Candida albicans is also responsible for causing jock itch
• Allergy to soap, washing powder, perfumes (chemical irritants) may also cause jock itch
• Senile pruritis – In old age the sebaceous glands in the skin becomes less active ,the skin looses its natural moisture and tends to become dry which leads to itching
• Jock itch may be contagious. It may be passed from one person to another by skin to skin contact our contact with unwashed clothing
• The presence of public lice, pthirus pubis can be one of the reasons
• Diabetes is marked by high levels of blood sugar which may give rise to groin itching
• Jock itch can also be triggered by friction from clothes and prolonged wetness in the groin area such as sweating
• Normally healthy skin secretes acid on its surface. This acid necessary for health and integrity of the skin, which also acts as a protective barrier. However through malnutrition, constant showers, harsh soaps, lotions and shampoos that removes the acid layer, toxins, poor blood circulation, low oxygen levels in the blood etc, health of the skin is compromised leaving it more vulnerable to fungal outgrowth.
• Eczema may be a cause of itchy groin, particularly in infants. It is described as the inflammation caused by a hypersensitive reaction in the skin. Eczema generally tends to run in families which often have a history of asthma or hay fever.
• Lichen Simplex is a skin condition that results in scratchy and itchy groin area. It may be accompanied with other skin disorders such as eczema and psoriasis.
• Allergic Reaction the chemical irritants such as detergents, body sprays, creams, ointments, fabric softeners, contraceptive foams or jellies can trigger an allergic reaction in your body. This may, in turn, lead to the formation of itchy, red rashes in groin area.

Examinations and Tests

Eczema, Lichen Simplex and Allergic Reaction are also reasons for if :-

The Doctor can usually diagnose jock itch based on how your skin looks.

Tests are usually not necessary. If tests are needed to confirm the diagnosis, either a culture or a skin lesion biopsy (for example, a scraping of the skin) may be done.

Complications :

• Permanent change in the skin colour of the affected area may occur.
• Secondary bacterial skin infections may occur.

Treatment

Topical treatment is of great importance in treating jock itch. This includes a variety of treatments available in form of creams, lotions, paints, shampoos and medicated powders. The treatment given is in accordance with the severity of the infection.

Mild fungal/jock itch is often treated by a combination of: –

• Washing the groin twice daily with antifungal shampoo.
• Using its topical antifungal cream.
• Severe jock itch is treated by combination of antifungal shampoo, antifungal cream and antifungal oral medicines.

AYURVEDIC TREATMENT FOR JOCK ITCH :-

First and foremost, the cause of itching must be ascertained. Without knowing this no treatment would be efficient. Based on the cause, follo wing Ayurvedic treatment can be prescribed:-

 If itiching is due to eczema, paradadi balm can be applied locally on the skin. For severe eczema RASA MANIKYA can be used. Also Dadrughna lepa gives excellent results.
 Shuddha Gandhaka or Gandhak rasayana is an internal medicine to be used with external agents
 Take few neem leaves and grind them with water to make a paste. Add some turmeric to it. Use this paste for local application to the affected area and let it dry for at least 2 hours. Jock itch usually responds promptly to treatment.
 For fungal infections in females YONI PICHU is a therapy whereby cleaned sterilized cotton is dipped in medicated oil and placed within the vaginal passage. The swab/ cloth roll should be retained for a long time so that it exhibits action locally.Ulcers if present heal, pain subsides and local hygiene improves. This treatment is particularly useful in case of a vaginal infections and can be done using Bala taila (oil), Ashwagandha taila (oil), Phala ghruta, Triphala ghrut etc.
 YONI PRAKSHALANAM is also ayurvedic therapy for females. By this method by Vaginal area is cleaned with medicated oils and decoctions. PRAKSHALANAM the means washing /cleansing. Thus it is a gentle cleansing measure of genital organs to avoid fungal or yeast infections.For this a decoction of Triphala or Dashmoola can be used.
 Another Ayurvedic effective therapy is YONI DHUPAN. Groin Area is exposed to fumes of medicinal herbs (dhup). Herbs such as Daru Haridra, Lodhra Turmeric, Mangistha, Sariva etc.

Some Ayurvedic medicines which work as blood purifiers are used for skin itching such as Kishor guggal, Raktadoshnta kadha, khadiraristham, Nimbadi Kashayam, Maha Manjisthadi Kadha etc.

Home Remedies

These are some simple home remedies which the patient should try for any skin ailments. It gives effective results and prevent further infections.

• Avoid overexertion and workout routines, which may cause excess sweating.
• Keep your genital and groin area clean and dry.
• Saltwater tub bath is one of the simplest and effective home remedies for jock itch. Sought had still trying the infection and also inhibits the spread and growth of fungi.
• Increase in take of Garlic, fruit and vegetables is supportive diet during infections.
• Have a cup of yoghurt daily.
• Change undergarments twice or thrice daily.
• Alcohol can be used as a topical application for treating jock itch but it aggravates the infection if taken internally and hence completely avoid alcohol intake during infection.
• A decoction of licorice when applied locally with a cotton swab is also effective
• Use coconut oil in the affected areas as often as possible
• Wash the groin with the diluted juice of a freshly squeezed lemon which can help to dry rash
• Keep your blood sugar level under control if you have Diabetes.
Avoid using substances that you suspect contain the chemical allergens.
• Do not share cloths, bath suits and towels with anyone.
Wear clothes after the groin area is completely dry after bathing.
• Lose weight in case you are overweight. This will provide you relief from itchy groin.
• Resist the urge to scratch or itch as they will only aggravate the condition.

Prevention

Since fungi tend to thrive in warm, dark, damp conditions, minimizing these conditions can help treat and prevent this rash. Some useful measures are:

1. Thoroughly clean the area is bit a hand-held shower and mild soap.
2. Keep your groin clean and dry at all times. If you are prone to jock itch, then cover your groin with antifungal or drying powders after you take a bath.
3. Avoid clothing or underwear that irritates the area. Choose loose fitting clothes with smooth fabrics that let your skin breath (like 100% cotton) rather than synthetic fibers. Wearing boxer underwear or no underwear, increasing air-flow by sleeping near a fan, wearing loose sleepwear or no sleepwear etc.
4. Exposing the area to wind and sun.
5. Take utmost care of personal hygiene
6. Refrain from scratching as it will irritate the rash and break the skin, creating the potential for infection.
7. Keep your blood sugar under control if you have diabetes
8. Do not share towels, hair brush, comb or clothes.

Premature Ejaculation (Early Discharge during Intercourse)
Often many men come to our clinic with complaint of early discharge to such extent that they cannot enjoy sex. Often the partners are also disturbed by this short duration of intercourse. We call it Premature Ejaculation (PE). The problem of premature ejaculation occurs either prior to or a few moments after sexual penetration, leading to unsatisfactory sex for both you and your partner.

What is Premature ejaculation?

You are having PE if you ejaculate within one minute or before penetration and thus unable to enjoy intercourse and may try to avoid intercourse due to this fear and you have lost ability to control the ejaculation. The problem may even occur during masturbation.



Premature ejaculation can be classified as lifelong (primary) or acquired (secondary). Lifelong premature ejaculation occurs all or nearly all of the time beginning with your first sexual encounters. Acquired premature ejaculation has the same symptoms but develops after you've had previous sexual experiences without ejaculatory problems. But whatever be the problem, it is easily treatable although for some men it can take some time.

What are the causes of PE?

It may be due to anxiety or stress. Many men with relationship problems with their partners or wives feel this problem. Nut make sure that it's not due to Erectile Dysfunction (ED). Men who are anxious about obtaining or maintaining an erection during sexual intercourse may form a pattern of rushing to ejaculate, which can be difficult to change. Smoking and alcohol can worsen the problems. Some other diseases may sometimes be responsible- like prostatitis (infection of the prostate gland), problems in nerves supplying the penis, problems in some hormones and rarely diabetes. So, it's important to pinpoint the cause so that the treatment can be started sooner and easily.



Is it a serious illness?

Unlike erectile dysfunction, PE is not usually related to any serious diseases (remember erectile dysfunction can be sign of early heart disease or diabetes). But it can affect your family relationship. In severe cases, it may be responsible for infertility.



What to do?

Initially you can try home-remedies. Avoid anxiety and stress. Reduce alcohol and stop smoking. Other simple strategies are avoiding intercourse for 2-3 days so that you can achieve better control in it. Alternatively you can try masturbation one or two hours before actual intercourse so that the discharge can be delayed. You can try squeeze and release technique (squeezing the penis when you feel urge to ejaculate). Using thick condoms can help some men by reducing sensitivity. If these do not work, you should consult your doctor.



Why should I consult doctor for PE?

There are advertisements of various untested drugs in newspaper and TV. If you try these, you can damage your sex organs or sex life as a whole. Because, we do not know what are the contents of these medicines or oil and how do they work. Moreover we do not know how far they are safe. So if home-strategies do not work, get rid of hesitancy and consult doctor. Your doctor may ask you some questions but please be free to reply to them, however embarrassing these might be. He may examine you to find out the cause of this PE. It is important to be sure that you are not having any ED, for which separate treatment is needed. Often counseling and talking to doctor help a lot. He may help you to perform some exercises and follow some tips properly. If you cannot do these exercises properly, these will be of no benefit. Finally your doctor may give you some drugs or ointments to improve your performance. It is important to follow doctor's advice properly- to take drugs in prescribed doses and timing. Usually men with secondary or acquired PE improve faster than men with primary PE.



Conclusion-

You and your partner have full right to enjoy sex for the duration as you want. Do not disappoint you and your partner. If you feel that you cannot control your early discharge, do not feel embarrassed. Consult doctor and get cured.

Contraception- Myths and Misconceptions
Today 26th september is the world contraceptive day. Sex is a basic human right. But to have sex safely and without fear of unwanted pregnancy should be the priority. Many women across the world die everyday as a result of unwanted pregnancy, mostly due to unsafe abortion.

So, couples can avoid pregnancy and enjoy sex life fully, only if they can follow safe and effective contraceptive advice. Let me clarify some popular myths and misconceptions abut contraception.

1. one contraception is useful for all

While the ideal contraceptive is necessary but it does not exist. So, what may seem suitable for a couple may not be useful for another couple. We need to assess various points like number of children they have, wish to have further children and plan to have children, general health of wife and husband (and any diseases they suffer from), frequency of intercourse, chance of infection, age of the partners etc.

2. Regarding barrier methods
Most popular barrier method is male condom. But the myths are that" condoms reduce sexual pleasure" and" condoms get easily damaged leading to failure.

Condoms are specially useful for those who are at risk of infections, who do not have very frequent intercourse and who are not suitable for other methods. Condoms do not decrease sexual pleasure, some of the condoms have medicated products which can increase the duration of intercourse and also act as lubricant to facilitate intimacy.

But men should know how to use it properly. It should be worn in erect penis expelling all the air and projecting the tip beyond the tip of penis. While withdrawing penis after intercourse, the condom should be held at the base of the penis. Actually failure/ rupture of condoms occur because of mishandling of condom or not knowing proper use of it. So, before use, please make sure that you know its correct use.

Definitely some men can feel latex allergy and they have to use non-latex condom, which are widely available.

There are barrier methods for females also- like female condom, diaphragm and cervical cap etc. But they are not very ease to use and are not so popular.

3. Oral contraceptive pills (ocp)

Myths-" ocp causes increase in weight" ocp causes cancers.

Facts- ocp seldom causes gain in weight. The gain in weight is coincidental, that means you have some other reason of gaining weight- like not having exercise, ignoring diet control and some other diseases.

Ocp rather reduces risk of cancer- especially ovarian cancer (which is very difficult to diagnose and very lethal), cancer of colon, cancer of endometrium (inner lining of uterus). Of course, there is little risk of breast cancer, but the risk is not very high, it can be easily detected by examination by doctor. So, if you take ocp, get rid of all the fears. Just try to have self- breast examination every month.

4. Intrauterine contraceptive devices (iucd)-

Most popular is copper t. Medicated iucds are also available, that contains hormonal drugs.

Copper t does not increase risk of infection, a common misconception lies among woman.

5. Emergency contraceptive pills (i pill etc).

Often couple uses these pills regularly after intercourse. Remember these pills contain very high content of hormonal drugs, which can lead to serious side effects like hormonal imbalance, irregularities in periods (and thus can prevent you from getting pregnant in future, even when you want pregnancy), ectopic pregnancy (pregnancy outside tubes that are life-threatening) etc. So, use regular contraception if you want to prevent pregnancy. I pill is not meant to be taken for every time of sex.

6. Ligation-

Ligation of male (vasectomy) and female (tubectomy) is done as permanent contraception for couples having more than 2 children. Ligation in male does not disturb sexual activity. It is easier than female ligation. Again, female ligation is also a very safe technique.

So, please remove all misconceptions and myths about contraception. Have safe sex, enjoy life.

Erectile Dysfunction
Erectile dysfunction (ed) occurs when the man is unable to achieve or maintain his erection of penis to meet his or his partner's sexual satisfaction.
The problem is often faced by newly married men. Fortunately with experience, most of them are able to overcome this problem. Again it is increasingly common with increased age.

So, what are the causes. In some male, it is psychological, caused by anxiety, depression, stress, marital disharmony etc. Use of alcohol and smoking will aggravate the problem. Psychological causes can happen at any age but more common at younger age. But in some cases, it is associated with serious diseases, especially at old age. It has been seen that in apparently healthy male ed is the first sign of heart disease. So, if you have ed, make sure that i's not caused by your heart problem. Again diabetes is another important cause. Excessive weight gain will also cause hormonal imbalance and thus problems in erection. Some hormone may also cause ed like thyroid, prolactin and even testosterone (male sex hormone). If you have chronic diseases like liver or kidney disease, it may also lead to ed. Diseases of nerves and spines should also be kept in mind. Sometimes, infections or tumour in private parts (prostate, testis etc) will also add to ed. Other diseases of penis like abnormalities in blood vessels can also be responsible, especially in young guys. Apart from these, some surgeries in abdomen and some drugs (like drugs to lower blood pressure, antidepressants, hormonal drugs etc) will also cause ed.

Now, what you can do? if you want to suffer silently, you will land up in other problems like problem in sexual life, problem in family relationships, depression and even inability to make your partner pregnant. But do all men with ed have to go to doctor? initially, the men should try life style modifications like avoiding smoking and alcohol; avoiding stress and anxiety; making good understanding with partner and also reducing weight. The partner should also have sympathetic attitude to the husband who is having ed.

But if still the issue is not resolved you have to consult doctor. It's not a mater of hesitation or embarrassment, as you may think. Rather it is needed to ensure that you are healthy. So, your doctor will talk to you, will check you and try to find out if any cause is there or not (like heart disease, diabetes, hormones etc). He may order some basic tests to ensure that everything is alright. After that he will advise you the treatment. If the cause is found, treatment of the cause (like diabetes) may cure ed. Even if no cause is found, ed can be treated by various ways.

But should you try the drugs shown in television or newspaper? remember, drugs used in ed have important side effects and we advise drugs only when it is absolutely necessary. In most cases simple measures like stress reduction, exercise help. Sex counseling of both the partners helps a lot. And if still ed persists, we advise drugs. There are variety of drugs, all are having own advantages and disadvantages. Particularly sildenafil group of drugs can affect heart and retina. So, we always practice caution before prescribing such drugs. And when patients are advised such drugs we advise to inform us when he feels eye problems, chest pain, fainting or even painful persistent erection of penis (" priapism").

There are other drugs that can be applied directly over penis. Testosterone group of drugs are prescribed only in selective cases as they have serious side effects and even they can reduce the size of your testes. In some cases where drugs cannot be given or drugs fail, we advise penile implants or artiificial pump to maintain erection. Finally in very few cases, where blood vessels of penis is blocked, we can cure it by some operations.

So, if you feel ed, don't get depressed. At the same time don't feel embarrassed to take medical help. Today's embarrassment may harm you in future. So, get cured and enjoy your personal life.

SEXUAL DYSFUNCTION IN MALES
Sexual dysfunction refers to a problem during any phase of the sexual response cycle that prevents the individual or couple from experiencing satisfaction from the sexual activity. The sexual response cycle has four phases: excitement, plateau, orgasm and resolution.

Causes of sexual dysfunction
Physical causes Many physical and/or medical conditions can cause problems with sexual function. These conditions include diabetes, heart and vascular disease, neurological disorders, hormonal imbalances, chronic diseases such as kidney or liver failure, and alcoholism and drug abuse. In addition, the side effects of certain medications, including some antidepressants drugs, can affect sexual desire and function. Psychological causes These include work-related stress and anxiety, concern about sexual performance, marital or relationship problems, depression, feelings of guilt, and the effects of a past sexual trauma.

The most common problems related to sexual dysfunction in men include ejaculation disorders, erectile dysfunction and inhibited sexual desire.

Ejaculation disorders
There are different types of ejaculation disorders, including:

Premature ejaculation
This refers to ejaculation that occurs before or soon after penetration.

Inhibited or retarded ejaculation
This is when ejaculation does not occur.

Retrograde ejaculation
This occurs when, at orgasm, the ejaculate is forced back into the bladder rather than through the urethra and out the end of the penis. In some cases, premature and inhibited ejaculation are caused by psychological factors, including a strict religious background that causes the person to view sex as sinful, a lack of attraction for a partner and past traumatic events . Premature ejaculation, the most common form of sexual dysfunction in men, often is due to performance anxiety during sex. However, organic causes are sometimes present. Certain drugs, including some anti-depressants, may impair ejaculation, as can nerve damage to the spinal cord or back. Retrograde ejaculation is most common in males with diabetes who suffer from diabetic neuropathy (nerve damage). This is due to problems with the nerves in the bladder and the bladder neck that allow the ejaculate to flow backward. In other men, retrograde ejaculation occurs after operations on the bladder neck or prostate, or after certain abdominal operations. In addition, certain medications, particularly those used to treat mood disorders, may cause problems with ejaculation.

Erectile dysfunction
Also known as impotence, erectile dysfunction is defined as the inability to attain and/or maintain an erection suitable for intercourse. Causes of erectile dysfunction include diseases affecting blood flow, such as atherosclerosis nerve disorders; psychological factors such as stress, depression and performance anxiety; and injury to the penis. Chronic illness, certain medications and a condition called Peyronie’s disease (scar tissue in the penis) also can cause erectile dysfunction.

Inhibited sexual desire (reduced libido)
Inhibited desire, or loss of libido, refers to a decrease in desire for or interest in sexual activity. Reduced libido can result from physical or psychological factors. It has been associated with low levels of the hormone testosterone. It also may be caused by psychological problems, such as anxiety and depression; medical illnesses such as diabetes and high blood pressure; certain medications, including some anti-depressants; and relationship difficulties.
Tests help evaluate sexual dysfunction include:
Blood tests These tests are done to evaluate hormone levels. Vascular assessment This involves an evaluation of the blood flow to the penis. A blockage in a blood vessel supplying blood to the penis may be contributing to erectile dysfunction. Sensory testing Particularly useful in evaluating the effects of diabetic neuropathy, sensory testing measures the strength of nerve impulses in a particular area of the body.

How is male sexual dysfunction treated?
Many cases of sexual dysfunction can be corrected by treating the underlying physical or psychological problems. Treatment strategies may include the following:
Homeopathic management

Human semen is naturally colonized by bacteria, and a new study suggests the microbes might have a role to play in both HIV transmission and levels in infected men.U.S. researchers found that bacteria in semen — the “microbiome” — play a role in local inflammation and in the production of HIV by infected men. They say the findings point to possible targets for reducing transmission of the AIDS-causing virus.
The study couldn’t prove that the bacteria was causing changes in HIV levels, and the researchers say more research is needed. However, the findings do “suggest an interaction between semen microbiome, local immunology and semen viral load,” wrote a team led by Lance Price of the Translational Genomics Research Institute in Phoenix.
Although HIV is found in many bodily fluids, the virus is most commonly spread through semen. In addition to sperm, semen naturally contains bacteria and chemical immune factors. One such immune factor is interleukin-1beta (IL-1b), which is involved in the body’s inflammatory responses.
“Higher bacterial load in semen could lead to higher IL-1b levels, which in turn could induce viral shedding [production], thereby increasing viral load,” the researchers theorize.
In the study, Price’s team analyzed semen samples from 49 gay or bisexual men. Of all the participants, 27 were infected with HIV. These men provided samples before they started antiretroviral drug therapy, as well as one six months later.
These samples were compared to 22 others taken from the men not infected with HIV.
The study, published July 24 in PLOS Pathogens, found that HIV infection appeared to alter the relationship between semen bacteria and immune factors. This affects viral load and could play a role in the sexual transmission of HIV, the team of scientists said.
Among the men with HIV who were not on antiretroviral therapy, the overall semen bacterial load correlated with their HIV viral load, the study found.
After examining the bacterial DNA in the semen samples of the men who did not have HIV, a total of 248 different types of bacteria were detected. On average, each sample contained 71 different strains of bacteria. However, immune factors were not tied to the amount of bacteria in the semen of these uninfected men.
In contrast, there was significantly less bacterial diversity in the semen samples taken from the HIV-positive men.
After taking six months of antiretroviral therapy however, semen viral load was reduced to undetectable levels. At the same time, bacterial diversity and semen composition became similar to the semen of the men who did not have HIV, the researchers said.

Every two years, researchers, implementers, policy makers, and community activists come together at the International AIDS Conference to take stock of the pandemic: Where are we now? Where have we been? Where are we heading? Discoveries are heralded and strategies dissected. There are always more questions than answers, but there is one question that needs to be answered at AIDS 2014 and beyond: Do we conssider fot diagnosis,care and treatment of key poor aafected persons like flying or commercial sex workers,stigmatised and criminalised members of LGBRT ,many fall prey to MEN to MEN SEX ,drug injectors,trangenders,migratory labors and even truck drivers who move from one place to another for lifehood and ultimatelyfall prey to unsafe sex?
On a basic level, the answer must be a resounding and unequivocal “YES!” Every human life counts. Every life has equal value. Yet, while an affirmative chorus may echo in the halls of the conference, easy rhetoric will not be enough.Data analysis by UNAIDS indicates that as many as half of all new HIV infections globally occur in key populations. This should come as no surprise. The disproportional concentration of the virus in these groups is hardly news.Though we are frequently reminded that we are in the era of evidence-based public health, data-driven decision-making, and performance-based metrics, the evidence on HIV vulnerability in key populations is routinely ignored. We aren’t even counted in many places. Surveillance fails to find us. Not surprisingly, funding for HIV services responsive to our needs remains slight.

On July 11, 2014, the World Health Organisation released a long-awaited and rapidly developed publication, Consolidated Guidelines on HIV Prevention, Diagnosis, Treatment and Care for Key Populations. It is an impressive document written and reviewed by a Who’s Who of experts working with and representing these groups.

There can be no doubt about the sincerity or good intentions of the guidelines’ authors, and this document has the potential to influence policy and practice globally. Yet questions persist in the willingness of institutions — governments, donors, development agencies and civil society — to embrace their fundamental responsibility to the health of key populations and invest accordingly in a sustained and broad-based effort to end the unremitting toll of HIV and AIDS on our lives.

MANIPUR CAG Report, said that in March 2007, about 25,919 HIV cases were reported but The count increased to 40,855 in March 2012, is of a great concern, over the increase which is of a serious magnitude. Despite spending Rs 43.39 crore for implementation of Targeted Intervention (TI) programme, there's an increase in HIV positive cases. This just reflects poor performance and unsatisfactory results in curbing new infections in high-risk individuals, said the report.
Although Manipur AIDS Control Society (MACS) has rebutted the CAG report that was presented in Manipur legislative assembly..MACS authority as well as Targeted Intervention Programme Centres running in the state that the figure that have been projected cannot be an indication of transmitting of new HIV positive cases, rather the increase of HIV positive cases are the contribution from old cases which have been concealed in view as stigmatisation and discrimination, also many HRG persons from the neighbouring country and state who have got tested in the state is also one of the factors in the increase of HIV positive population.
Six countries – China, India, Indonesia, Myanmar, Thailand, and Vietnam account for more than 90% of the people living with HIV in the region.
'India has the third largest number of people living with HIV in the world – 2.1 million at the end of 2013 – and accounts for about 4 out of 10 people living with HIV in the region,' the report said.

It said HIV treatment coverage is only 36% in India, where 51% of AIDS-related deaths occur. The report said new HIV infections in South and South-East Asia declined by 8% and by 16% in the Pacific between 2005 and 2013.In India, the numbers of new HIV infections declined by 19%, yet it still accounted for 38% of all new HIV infections in the region. The proportions of people who do not have access to antiretroviral therapy treatment are 64% in India.
