The Concept of day care started in India only some 2000 years back with Susruta.The Recent advances in the endoscopy, optics and newer generation of lithotripters better imaging modalities have made the urology more patient friendly. More and more procedures can now routinely be done on Day care basis. The advancements in anesthesia techniques have also made the patient recovery smoother without the routine post-operative problems like drowsiness, nausea and vomiting. The continuous epidural analgesia can also allow patient to be active and watchful of own surgery on the video monitor. The surgeon can chat with the patient and make the patient well versed with the steps. Many times in the surgical practice we have seen patient losing all the surgical records and that leaves the surgeon in situation where he has to get more and more costly investigations for availing the details of the patients condition. But you can well imagine a patient coming to and saying I have lost the record but I know exactly my condition as I was actively interacting with the surgical team before, during as well as after the procedure. The day care surgery has advantage both for the patient and the hospital. The patient goes home early after the surgery-recuperation is always good in home setting, economical (cost of hospitalization is markedly reduced) and there is less risk of acquiring the hospital infections. The hospital has the advantage of reducing the incidence of hospital acquired infection. The day care option is especially exercised for the patients who stay near by the Hospital premises and with the telephonic communication facilities as if the patient needs any medical assistance after the discharge he can either call up the doctor or visit the hospital. The commonly performed surgeries in adult patients are diagnostic urethroscopy, VIU, diagnostic cystoscopies, cystolithotripsy, hydrodistension, RGP, DJstenting, DJ stent removal, TURBT( endoscopic treatment of bladder tumours), Botox injections.Ureteroscopies, circumcision, varicocelectomy, orchiectomy, orchiopexy, vasectomy and reversal of vasectomy, arterio-venous fistulas are also performed commonly in office set-up.The paediatric surgeries like paediatric cystoscopies, PUV fulgurations, paediatric ureterorenoscopies, STING procedure (submucosal teflon/ deflux injection) for Vesico-Ureteric Reflux. The ambulatory set-up for the day care urology needs expert team of doctors anaesthetist, nurses and technicians. The instruments needed are well equipped endourology Operation Theatre with C-arm, Lithotriptor, flexible endoscopy and laparoscopic set. The advent of LASER has revolutionized the management of may urological surgeries in day care setting. Holmium Laser uses 60-100 W power and a 200-550 μ fibre.It works by the principle of photoacoustic / photothermal phenomenon. Some of the procedures that can be done by Holmium LASER applications in day care are management of stones,small prostate ,urethral stricture etc. Holmium Laser Enucleation of Prostate (HoLEP) enables relatively blood less surgery, brief catheterisation and hospital stay and immediate symptomatic improvement. It is also safe in patients on anticoagulants, patients with cardiac ailments or coagulopathies. The R.G. Stone Urology and Laparoscopy Hospital treats around 70% cases by Day care Treatment. The Day care Operation Theatre Suite is well equipped with the Siemens Multimodularis state of the art Lithotriptor with the ultrasonic localization, Karl- Storz endoscopy and Laparoscopy instruments, Holmium-Pneumatic and Ultrasonic intracorporeal Lithotripsy for the patients of kidney and Gall Bladder calculus. The patients with prostatic enlargement and urethral stricture are treated with Holmium 100W Luminus LASER machine. This LASER machine is also used for the treatment of congenital obstruction in genitourinary tract. We have dedicated surgical and nursing staff for the day care facility which include Dr.Naveenchandra Acharya, (Urologist), Dr. Hemant Chhajed (Laparoscopic surgeon)and Dr.Sonali Chaturvedi (Anaesthesiologist).
