Three major variables namely; selection of implant,skill of the surgeon & selection of the patients, affect the outcome. Market is flooded with varieties of implants.which implant to choose? Meta Analysis of available literature shows, Charnley's cemented total hip system has longest survival in terms of survivorship analysis. Surgeons using this system needs to be trained in its surgical technique as advocated by Charnley,himself.It should be performed in "army like precision" with special instruments designed for this implant. All patients & their underlying hip diseases are not always the same.Patients should be counselled for the same prior to undertaking this major surgery.
